MEET THE TEAM

The Board
Made up of member practices consisting of General Practitioners, Practice Managers and a nurse. The board sets the network’s strategic objectives in conjunction with the CEO and monitors progress towards achieving these objectives in terms of overall delivery of care services. 

Chief Executive Officer
Ashraf Ullah
Is responsible for the success of East End Health Network. The Board and the CEO assures East End Health Network successfully delivers Network Improved Services (e.g Long Terms Conditions Management) to the local community, the accomplishment of East End Health Network’s mission and vision, and accountability to East End Health Network's diverse stakeholders. He is responsible for the management of the company, day-to-day operations and the organisational development of EEHN. The CEO provides strategic direction enabling the Board to achieve its aims and objectives in line with its vision. This also includes managing organisational governance.

​Operations Manager
Ashadul Alom
Ensures the Network's data is managed and updated in line with its strategic management plan and operational objectives. He is responsible for the planning and supervision of all the operational functions of the company. Contributes to the development of strategic plans and performance objectives set forth by the Network. Undertakes regular audits and analysis to ensure data integrity and inform operational and strategic management of performance.

​PCN MDT & Access Facilitator
Aysha Begum
Is in charge of supporting the delivery of the Network's care services for example through the centralised call & recall system. Supports the organisations needs by facilitating and running meetings, programmes and special training events for example Multidisciplinary Team Meetings (MDT's) and educational sessions. Also assists with the coordination of community engagement and other corporate programs helping the Networks communication plan, as well as supporting the Operations Manager with updating IT systems. 

​Senior Recall Administrator
Shumina Begum
Shumina Begum our Senior Administrator is responsible for the management, administration and delivery of all care package calls and recalls and services in order to ensure that they are accomplished in an effective and efficient manner. Shumina has direct contact with our patient population on a day to day basis and schedules all patient appointments for the recall team. Shumina is also the first point of contact for our Chronic Disease patients.

Recall Administrators
Kaniz Era & Kolsuma Khatun
Kaniz Era and Kolsuma Khatun are our Recall Administrators and in this capacity they both assist the practices with their individual patient population recall lists while ensuring the networks recall list is also being attended to, both also have direct contact with patients across all practices and enable patients to book their appointments with ease. Kaniz and Kolsuma are also the first point of contact for our Chronic Disease patients.

Chronic Disease Nurse
Abiola Dada
Our Chronic Disease nurse undertakes a range of nursing assessments and provides appropriate care/treatment in conjunction with GPs, practice nurses or independently as appropriate for the need, according to practice policy, protocols etc. 
As our Diabetic Lead she interacts with most of our Type 2 Diabetic patients and has developed a personalised way of delivering care to our patient population across all of our practices. Our nurse always strives to improve standards of care and is involved in identifying practice population needs.

Network Pharmacist
Rajvir Khinda
Our Network Pharmacist Rajvir supports the practices with medication requests from patients. 

​Community Link Workers - Social Prescribers
Shamimuz Zaman & Rebeka Chowdhury
Shamim and Rebeka, the Networks Community Link Workers enable GP's, nurse's or other healthcare professional's to refer patients for support to find extra services. This is to help improve health and wellbeing and to make positive changes. They both can link people with many services in the local community that can help. These include:
  • Housing, benefits and financial support
  • Employment, training and volunteering
  • Education and learning
  • Social activities
  • Healthy lifestyles and active lives
  • Arts, gardening, creative activities
  • Befriending, counselling and other support groups
