Consent to share your Care Information
In order to offer people who have more complex care needs and more coordinated health and social care we need to be able to share medical information between organisations that provide health and social care services. This information will only be used by health and care professionals to improve care. It will not be used for any other purpose.
Health and care professionals in Tower Hamlets are now working more closely with your GP and also with families and carers so that people with more complex health and care needs are better supported and any future hospital stay is hopefully avoided or at least shortened.
In order to prevent hospital admissions and to keep people healthier we need the consent of individuals and/or their guardians to share medical and care information confidentially with colleagues in other health and care organisations (e.g. hospitals, emergency services and adult social care team).
Below is a form asking you to consent to share information between certain organisations to improve care, once you have completed this form a member of staff from your doctors surgery will add this information to your record which will enable the surgery to share medical and care information confidentially with care organisations.
NO information will be shared with other agencies such as: housing provider/landlord, voluntary agencies or the Department for Work and Pensions.

This message has been sent from your Doctors Surgery on Behalf of your named GP. Your doctor would like you to consent for outside agency such as Hospitals, District Nurse to look at your records. They will only able to see significant information like the medication you will be on or any allergies you may have and recent blood test results, It will not be used for any other purpose. They will not be able to see anything discussed with the doctor which has been put in as free text. Completing this form means you have consented to share your information, in doing this you are enabling a member of your doctors surgery to initiate sharing of your medical and care information confidentially with care organisations. |
